How To Design An Effective "Appraisal Policy"? Please Suggest!!

Dear All Friends:-P
I want to know that how to proceed to design an effective appraisal policy actually in our company appraisal system is not in place for this I have given this responsibility to prepare a new appraisal policy therefore I would request you all to help me in designing appraisl policy by giving your valuable suggestions/inputs like concepts to be taken into account.
If possible can any one please attach a sample format to help me in designing policy:?:

Hi Anu,
To create an effective appraisal system, you require the following as background:
1.Set KRA for each position, for this you require Job Descriptions.
2.All the supervisors should get a buy in from their respective subordinates for their specific KRA for the agreed period.
3.Once this is ready, you can set pointers for each KRA. Its better to have only between 3 to 4 KRA for each period.Pointers could be Quality of the output, Time, Errors & Discipline to name a few, you can decide on this.
4.Confirm on the periodicity of the appraisal cycle.
5.Create a template.
6.Give sufficient time for the employee & the supervisor to complete but not more than 10 days is required unless the supervisor has more than 50 direct reportees.
7.Consolidate the same with your recommendation.
8.Meanwhile confirm that the employee has accepted the ratings given by the supervisor.
9.Recommend as per the company policy for incentives or promotions.

Even I am trying to improve the appraisal form in my Company.... It a small IT Company. All this information provided by you guys helps me. I also wants to know if any one can help me on how the % will be determined to each filled form.
