ESI Form Submission Confusion: Do We Need Four Copies or Just One?

Dear All,

Please clarify whether we need to submit Form 5 in Quadruplicate or one copy to the ESI Office as per the latest amendments in the ESI Act.

I would appreciate a prompt reply.

Greetings.

You have to retain one copy of Form 5 and send quadruplicate copies to the nearest ESI (Branch) local office.

Form 5 is for half-yearly returns. You have to mention the Employee Insurance Number, Employees Numbers, 6-month salary amount, amount of contribution, ESI Hospital Name of the Employees preferred, etc.

You have to acknowledge the return in Form 5 in one copy for yourself and submit quadruplicate copies to your local office before the 11th of November, 2010, along with all six challans.
