Need Help Crafting a Transfer Request Letter to Move Closer to Family?

I need a formal letter format for requesting a transfer. I have been working in Port Blair, Andaman for the past one and a half years, and my hometown is in Madhya Pradesh (MP). I would like to request a transfer to locations in MP, Uttar Pradesh (UP), and Northern India. Could you please provide me with a sample format for this request? Thank you.
