Struggling to Get PF Slips for 5 Years: How Can I Request Them from the PF Office?

This is Rajesh, working as an HR Executive in one of the print media companies for the past two years. I have a problem - we have not been receiving PF slips for the last five years when we submit our annual returns each year. Can you suggest a format or procedure for requesting PF slips from the PF office?
