Performance Appraisal Form For Pharma Sector

Need help and pointers for designing a performance appraisal forn for pharma sector.

Dear Roma,
You do not seem to have taken my advise and have not given much detail. What do you mean by pharma sector? For category of workers and functional areas are you considering. Thouhg the standard format may be the same for all levels, the Key Performance Indicators differ.

The form is supposed to be for all levels of management cadre across departments i.e.sales, procurement,HR, Marketing and manufacturing etc.
