How Can I Create an Effective Questionnaire to Understand Employee Views on Training Programs?

Hi, I'm an MBA student currently working on a project in a retail MNC. My project title is "Employees' Perception on Training Programs in the Organization."

Objective of the project:
- To find out how much employees perceive the training programs conducted as effective and beneficial.
- To determine what type of training they need to perform their jobs better in the organization.
- To identify the problems faced by employees and suggest what the organization should do to make the training programs more effective, motivating employees to attend and learn.

Please help in formulating the questionnaire:
- What type of questions do I need to use to extract exact information?
- Are there any readily available questionnaires that I can use?
