Need Help Designing a Leave Card: How Do You Track Earned Leave in Your Company?

Hi all,

I am in the process of preparing a leave card for the employees of the company. Can anyone help me with a leave card format? If anyone is using a leave card for their own company, can they attach a copy of it? Our leave policy is that we have an annual 25 days' leave, but we provide leaves to employees on an earned leave basis, i.e., 2 days per month. How should this be shown on the leave card? What columns should be included in the leave card?

Dear Sara,

The Leave Card form usually includes the following:
- Name of Employee
- Designation
- Grade
- Basic pay
- Department
- Employee Code

The following should be present in tabular form:
1. Serial Number
2. Type of Leave
3. From
4. To
5. Leave Due for the Year
6. Accumulated Leave
7. Adjustments: If Due Leave for a Year Exceeds 25
8. Total Accumulated Leave
9. Special Leave: For Leaves like Maternity, Bandh, etc.
10. Remarks

You can also refer to the Leave Card under the Factories Act, 1948.

There should be provision for sanctioning or non-sanctioning. As mentioned by Swastic, please refer to the Factories Act.

Dear JSN,

Unless leave is sanctioned, it is not recorded in a Leave Card. Some companies keep a column where Absent/LOP is recorded. Additionally, some companies record Absent/LOP in red ink under the heading "Type of Leave."
