Struggling to Upload ESI Form 5 XLS? Can Someone Help Share It Here?

Hi, After searching CiteHR and Google, I spent around 1 hour formatting this XLS. I hope it is helpful. I did not check the contents and grabbed the same from ESI-Returns Form 5.doc. Please check and use the same.

I am unable to upload the file. I have tried a number of times. I can email the file to someone who can upload it on this thread.
