Sample filled forms of Form 10C, Form 19?

Anyone of u pls send me sample filled forms of Form 10C, Form 19
pls fill up any faulty info..i jus want to get an idea how to fill up these
forms...I have to apply for PF
Actually i have an account in ABN Amro bank.

this is a forum we can share our ideas ,it is very difficult fill up simple form and circulate it .it is a simple form reqd some personal information,one should know how to fill a form,if ant difficulty can be discussed.

Dear Sneha,
You can collect the forms from the nearest PF office - on production of your pf number.
The forms are normally in English and local languages - it is easy to fill up the forms - nothing to worry.
After filling up the forms you attach a xerox copy of your Savings Bank Account Passbook / statement which depicting the Account number clearly.
