Need Help Understanding How to Fill Out Form 10C and Form 19 for PF Application?

Could any of you please send me sample filled forms of Form 10C and Form 19? Please fill in any incorrect information. I just want to get an idea of how to fill out these forms as I need to apply for PF.

You can collect the forms from the nearest PF office upon production of your PF number. The forms are typically available in English and local languages, making it easy to fill them out - there's nothing to worry about. After completing the forms, remember to attach a photocopy of your Savings Bank Account Passbook/statement clearly showing the account number.
