PAN Card - Income Tax

Dear sir,

i lost my PAN card. I knew the PAN no and also holding the letter from the income tax department while alloting the PAN no. Kindly let me know how to apply for a duplicate PAN card

psl write to Income Tax Pan Services Unit.
Add:- Income TAX PAN Services UNit
Managed by UTI Technology Services Ltd.
Plot No.-3, Sec-11, Post Bag No.20, CBD Belapur, Navi MUmbai-400614.

and pls consult with Axis bank from u have made ur pan card.
