Actually, the file has not been uploaded. However, you can obtain the forms from www.epfindia.com and erlaws.com (in Word format).

Hi Chandoo,

You will get the PF withdrawal form from EPFO. Download 2 forms:
1. Form 19: Employees' Provident Fund Scheme, 1952
2. Form 10-C Pension: Employees' Pension Scheme, 1995

If you have worked in a company for more than 6 months, then submit both the forms; otherwise, submit only Form 19. An employee doesn't get the pension amount if he/she has worked in a company for less than 6 months.
