Attendance Sheet Formate Including Overtime Column

Dear all,
could you send me the attendance format including over time column.because where i work there only those employees are geting the Over time whose salary is below 5000/-. if those emplyees whose salary is above 5000/- in that case if he will do OT then he will adjust that OT by taking compensatory off.
so that iam very confuse that how we will adjust their compensatory off & how we will find out the Total OT whose salary is less than 5000/- in a single attendance formate ? what formula we will put in that attendance sheet that automatically it will calculate.

Pls don't get confused,
formula for calulating OVerime is
= Gross salary p.m./26/8*OT hrs*2
It is given double from the double rate, ESI is deducted on this and u can give max overtime to an employee of 51 hrs in a quarter above the cap is against the law.
for a attendance format, m attaching a format for ur reference.
OR do one thing make a separate sheet of people whose salary is less than 5000 named overtime calculation.
