Seeking Help: What Questions Should I Ask HR and Employees About the Global Crisis?

I am working on a project on the role of HR in the recent global crisis and am in search of a good questionnaire to target both HR and employees. Please assist in developing questions that suit the current scenario so that the results are optimal.
