persently i am working in HR deptt. at panchkula & i just want to that how we can change our ESIC dispensary chandigarh to panchkula becouse our current ESIC dispensary at in chandigarh & we want to change it.
kindly provide exact detail with which document/form is required & processor.

For the establishment there is no such dispensary but it is for the employees a dispensary is allotted. Therefore, if any employee requires to change his dispensary, a request to change dispensary in the prescribed form ( I do not remember the exact form) shall be made to the Local Office of ESI Corporation. Please contact the Manager, ESI Local Office for details.

Dear Vikash,
as Madhu sir said if u want to change the dispensory of any employee use form 53. or else u want to change local esi office, in that case u have to give application to both the office current local office & new local office for tranferring ur records.
for more on this pls consult with esi people or u must have contacts with any esi official.

the cap for ESI is rs10000/- gross salary P.m.
Employee Contr-1.75%of Gross salary
Emplyer Contr-4.75%of Gross salary
