Questionnaire For MBA Project

Hi can any one help me out in preparing the questionnaire for my MBA Project. The topic i have choosen for my project is Non monetary rewards. I hav no idea of how to prepare a questionnaire can anyone help me plz.... If any one can give me suggestions or some models of questionnaire it vl b very useful for me plz... Its URGENT as such...

Could you give me some more info about where you are studying, whether you have attended a module on Research methodology, do you have a project supervisor in your institution who is supposed to guide you, when do you have to complete the project, etc?
