Need Form A for Bombay Shops and Establishment Act Registration – Can Anyone Help?

I wanted Form A for registration under the Bombay Shops and Establishment Act, 1948. Please provide me the same.

You can get the said form from the BMC site or go to any stationary shop asking for Form A. It is also available at your nearest BMC ward office.
