I am an HR faculty and physician. I suggest that you prepare a questionnaire on the following points:

- Occupancy capacity of the hospital
- Work shifts
- Shift hours
- Nurses/patient ratio
- Organizational culture
- Pay scales
- Promotion opportunities
- Duration of service
- Relations between management and employees
- Absenteeism, etc.
