How Can I Effectively Measure Employee Satisfaction with Our Welfare Activities?

I want to find out the satisfaction level of the employees with respect to the current employee welfare activities. Can anyone tell me what needs to be included in the questionnaire? What kind of questions? How exactly can I measure the satisfaction level? This is with respect to a project I need to know urgently.

Dear Jean,

I am willing to help you, but I do not have questionnaires to send. Are you studying full-time? Have you not got a tutor guide? Where will you be doing the study, and how many interns are there? Have you done any literature survey? If not, you need to do that first to establish the Research Framework. I have been conducting a literature survey for the past two months on a project, only to find out that my supervisors disapprove of it. That's why I have pivoted the topic in a different direction, especially since my guide is interested in this new approach. However, a drawback is that the current interns will be leaving by next week, and conducting a survey on new interns may not be the best choice.
