Name Change After Marriage: What Documents Are Needed for HR and PF Records?

Please help me with the procedure to change the name after marriage for women. Which documents do we need to ask the employee for when changing the name in our records as well as in the PF office?

The employee needs to submit an affidavit notarized by a notary stating the change in name, and the same will hold good for the company to effect the changes in the records.

You have to approach the Registrar of Marriage and file papers. Then, apply for a change of name after marriage in the government notification published in the Official Gazette.

I just want to add on to what Badlooser has said, that employees should give a letter requesting a change of name along with the marriage certificate and a copy of the notification received from the Official Gazette. For a name change in the PF, the PF nomination form should be revised, filled, and submitted to the PF along with the covering letter and a copy of the marriage certificate. The same procedure can be done for a change in the name in ESIC by filling up the required form and submitting the necessary documents.
