Hi Veena,
I am sending you the format of ID card used in my organisation.
On the front:
Companies name with the companies logo
Employees photograph Name of the employee
Designation
Location
Blood Group
Employee ID
Signature of the issuing authority
Companies Address
On the back side of our ID card, only a sentence is written. It's "This card is the property of the issuer and must be returned upon request".

1. The card to be exibited on person while on duty.
2. Loss to be intimated to Chief Security Officer within 24hrs.
3. If found please return to :
Chief Security Officer
<Company Name>
<company Address>

1. card must be carried while on duty & presented on demand.
Nobody will be allowed to enter the premises without the card.
If found without the card on duty, strict action shall be taken against him/her.
if the card is stolen / spoiled pls. inform the concerned authority immediately.
Penalty of rs. 300/- will be charged for issuing a duplicate card.

This formate is very good , But one more thing i would like say .
we have to add Employee code and Validity date for Id cards because this will help us review new information and the card will be very good Id proof to employees.
