How Do I Create an Effective Candidate Evaluation Sheet for Interviews?

Dear All,

I need to create an evaluation sheet for candidates attending interviews for different positions so that we can compare the shortlisted candidates and select the best candidate for the position.

The evaluation sheet should flow from HR to the Line Manager and then up to the Director. This will ensure that the next interviewer can assess the candidates appropriately and ask relevant questions.

Please assist me with the format for such an evaluation.
