How Do I Surrender an ESIC Subcode for a Closed Factory in Thane, Mumbai?

I request you to kindly provide me with the process and documents required for surrendering the ESIC subcode in Thane, Mumbai.

In June 2000, we completed a project in the Thane area. For this, we obtained an ESIC Subcode from the Thane ESIC for one of our factories located in Kolkata. However, the code coverage letter was received in June 2007. Meanwhile, the aforementioned Kolkata factory has also been closed.

You can write the letter for surrendering the ESI Sub code to the ESI Office and contributions if you have to be paid in the same code number as you have received.
