We generally gather the background information before the finalization of the candidate, and upon joining, we insist on filling out the details in the Employee Information Sheet.

Just an afterthought... Is sibling name, DOB, and occupation really required in an Employee Info form? If yes, why?

Why do you need weight, eye color, and hair color? Eye and hair color can be changed these days (contact lenses and hair dyes), and you have a color photograph of the person to identify these details. Weight is not relevant in most industries unless you work as an air hostess or in some such occupation. Also, it is variable and highly intrusive into one's privacy. Even height is not relevant unless it is a work-related criterion.

Otherwise, Don, your employee information form is really compact and concise. Good work.
