Dear Mr. Ramesh,

It has been observed that you have not been coming to duty since 15-11-2012. You have remained absent from duty without providing any information or obtaining approval. You are aware that such unauthorized absence amounts to misconduct as per the rules of the Company.

You are therefore intimated to report for duty within 5 days from the receipt of this notice and explain the reasons for your unauthorized absence. If you fail to report for duty within the stipulated time without a satisfactory explanation for your absence, appropriate action, as deemed necessary, will be taken against you.
