Require Real Life CASES- BUSINESS COMMUNICATION

I need a help from all of you.
I require real life cases from your organisation, institution or classroom related to BUSINESS COMMUNICATION.
These cases are required to do a research work, it can be related to Communication gaps, miscommunicating, chosing the wrong channel of communication, open communication and methods.

Going by your suggested areas, are you looking for only "communication mishaps and subsequent rectifiation" or good communication practices being followed. I feel you should have both models to compliment your research. Pls clarify this on this to the group to take on further...
