Confused About Getting an SSN for PF Withdrawal? Need Help with Forms and Process!

Greetings.

If I am not wrong, we need to have a Social Security Number (SSN) before applying to withdraw PF. So, I wanted your help with 2 queries:

1. From where can I get the form as I have already searched the EPF Website and could not find it?
2. Where is the form supposed to be submitted? (e.g., my office is in Gurgaon, HR, so can I submit the form in Delhi PF Office?)
3. How much time does it take to get the SSN once the form has been submitted, or what is the procedure?

Hi,

The requirement of submitting the SSN form along with the PF withdrawal has been removed effective July '08. I have attached the notice regarding this change.
