Need The Ssn Form & Also A Query. - DOC Download

Hello all
Greetings
If i am not wrong; we need to have a SSN # before applying to withdraw PF.
So i wanted your help with 2 queries?
1. From where can i get the form as i have already searched the EPF Website and could not find out?
2. Where is the form supposed to be submitted (e.g. my office is in Gurgaon,HR so can i submit the form in Delhi PF Office?)
3. How much time it takes to get the SSN # once the form has been submitted or what is the procedure?

Hi,
The mandatory of submitting SSN form along with PF withdrawal has been removed effective July'08.
Attached the Notice in this regard.
