The form is ok. but there is a scope of improvement in "for offiece use only" section.
You can separate the leaves availed and balance leave as per the types of leave you have in your company.

IS it necessary to have a section for Office use in the Leave application form? I ask because all the leave details are maintained in the soft copies with Admin / HR team.
