How Can We Create a Survey to Understand and Improve Our Company's Culture?

My name is Murli, and I work as a Senior Executive - HR with a charter company. Our company is around 4 years old with 200 employees. We are planning to change the culture of the company. Currently, the company has no set guidelines or policy manuals, and we are in the process of developing them.

I would greatly appreciate it if someone could assist me in developing a questionnaire. We aim to start with a survey within our organization to understand how our employees perceive the current culture and what the ideal culture should be.
