Struggling with Needs Analysis Questionnaire for Manufacturing Workers—Can You Help?

I am currently pursuing my 3rd semester of MBA and am facing some challenges in constructing a needs analysis questionnaire for workers in a manufacturing company. Could anyone please help me out? Thank you.

Workers are the operatives in any manufacturing industry, and they are responsible for all operations, so it is a very sensitive issue to design their needs analysis.

In my opinion and experience, there are some things which you should keep in mind while designing TNA:
- Quality of work (rejection rate)
- Attendance & punctuality
- Initiative in work
- Ability to improve
- Cooperation with others

You can design any format consisting of these standard criteria.
