I need to withdraw money from my Provident Fund. It has been 11 months since I left my service. I resigned due to marriage reasons. Now, I need to fill out form 10-C and have some doubts:

1. What is PPO?
2. What do wages mean? Does it refer to the Basic salary?
3. What does the period of non-contributory service mean?
4. Can I send a scanned copy of the filled form to my relatives, and can they print it out and send it to the company? Is the scanned copy acceptable?

Hi,

In your case, since the service rendered is only 11 months, the PPO number is not applicable. This is applicable to those employed in service before 1995 under the EPS scheme.

Wages, in PF terminology, means your Basic + DA (if applicable), on which PF deduction is made.

The period of non-contributory service refers to any break in service continuity.

Lastly, please note that only originals will be accepted by any PF office; in your case, a scanned copy will not suffice.
