Need Help Designing an Effective Employee Induction Exit Assessment Form – Any Suggestions?

I am currently working in a manufacturing company. One of my current assignments is about measuring the effectiveness of Employee Induction, and I have to come up with a form for an Induction exit gate assessment. It would be a huge favor if anyone could help me out with this.

I am a newly joined employee in a shipping company and have undergone induction training. I have been taken to all the divisions of the office to learn about the activities carried out in each department and to get familiarized with the people. I have been requested to prepare a report summarizing my understanding of the activities in each unit. It would be greatly appreciated if you could provide me with details on how to prepare a report after induction.
