If you prepare a proforma salary slip on your own, who will sign them? Without the signature of the payroll officer, I don't think any employer would recognize this as an authentic document.

On the other hand, if you have their concurrence to prepare and your payroll officer can sign those slips, then it may be possible. If this is okay, then you may adopt any of the attached formats convenient to you for the purpose. Don't try to fake those slips and produce them. If they verify the authenticity, you will get caught. Be careful.
