Seeking Manager-Approved Training Effectiveness Format: Can You Help?

I need a format of Training effectiveness format. Please note this effectiveness will be done by the Managers of employees who have attended the training and not the participants. So if someone has a similar format please let me know.

Hi Sanjana,

I have recently come across an evaluation form from a reliable Training and Evaluation Site called Competency Analysis which may be very helpful to you. This will be answered by the participant's supervisors before, after, and a few months after the training has been conducted. The form is actually comprehensive, the directions are clear, and you won't have a problem understanding it.

You may want to revise the questions that cater to the current training program that you will be conducting. This was done for an Internal Customer Service Workshop that I recently handled. I hope this will prove helpful. I have attached a copy in Excel form for your reference.
