Seeking Simple Appraisal Forms for Non-Managerial Staff – Any Suggestions?

Could someone please send me appraisal forms or guidelines? I need simple ones as I have to deal with employees who are not very educated, such as office boys and documentation staff. I am new to this part of HR.
