The attachment is a format of PMS in my organization. I hope you find it helpful. I'll be putting more as soon as I get a new one. Some files may be corrupted. Please see the PMS section of Cite HR where I have posted an attachment.
Your forms are really practical. One thing I still do not understand is, in the PMS format, what am I supposed to put in "Measurement Criteria" and in "Weightage"?
Please guide me.
