I'm doing my MBA.now i'm insearch of sample questionnaire for my project in performance appraisal in manufacturing sector.can anyone help me or sent me any questionnaire

the attacment is a format of PMS in my organisation hope you find helpful, ill be putting more as soon as i get new one.

Your forms are really practical. One thing still I did not understand is, in PMS format, what I supposed put in "Measurement Criteria" and in "Weightage"?
