Need Help with PPE Record Format for Labor – Can Anyone Share a Template?

Dear All, can anyone give me the format of a PPE record of issue? I need that kind of record for labor.

You can use the attached format for maintaining the record of PPE issued. Open a register and dedicate one page for each worker.

The format is very good. The only suggestion is - if we are going to allocate one page per employee, the employee's name can be placed at the top of the page instead of in the 2nd column.
