PF Withdrawal Rule Changes (Recent)

Dear Nirav,
As per recent circular sent by PF office, RPFC pune has relaxed to settlement up to 30/04/2016 with permission from compitent authority.
The un-employment should be two month before application. If you are in employment while doing application, you cannot withdraw.

My core question is - Using what forms do I withdraw atleast my employee contribution, after May 1st ? I m based in Gujarat & my EPFO is Chennai.
