You approach your employer. He will submit Form 5 & 10 to your RPFC. Your date of joining and date of leaving, it seems, is not recorded with RPFC and therefore RPFC is asking you to submit Form 5 & 10 duly certified by your employer.
Currently Form 5 & 10 is not required to submit since the information is given through ECR. It seems your is a old case wherein your details are missing in system.
