If the company has enrolled you under ESIC benefit then it becomes the foremost duty of a company or an HR person to avail you the ESIC card and if they are not doing this then report to the authority.
hope you have done your registration properly.
once you complete the process of ESIC they will give you the smart card.

Dear Sheran,
To avail ESI benefits, your HR person/employer should help you out.
Who wants to avail the benefits? is it you or your dependants?
If you want your parents to get the ESIC benifits then you need to submit an income proof duly signed by your company's official. Also, it would be better if you can get an post card size photograph of yourself along with your dependants and get it signed along with the seal by your employer(authorised signatory) at the back side of the picture.
Ensure with the help of your employer that your details are updated in the ESIC site. The updation part is the sole responsibility of your employer.
From your query it is assumed that you perhaps dont have an ESIC card. If that is the case, then please ask your employer to provide you with a temporary identity card (TIC). After getting the TIC, first you need to sign and then the company's authorised signatory should also sign along with the seal.
Also collect the entitlement benefits certificate along with the TIC and photo.
You can get the name and address of your dispensary from your TIC and proceed accordingly.
Thank You,
Ramya Ranjan
