I want to do a Employee survey in the organisation (Junior & Senior staff members), please suggest how to start it and from where i can get the questionnaire for the same. (Site or any link)

You have to decide first whether your interest is to conduct 'Employee Engagement Survey' or the 'Employee Satisfaction Survey'. In either case, you ought to be clear about the abridged definition of 'Satisfaction' and 'Engagement' as applied to your company. To clear it further, there may be 20 parameters which constitute employee satisfaction, but for which 5 - 6 parameters your management will like to obtain feed back is an important consideration? This helps in the design and roll out of improvement initiatives which are fully mapped & aligned to the organizational objectives and are thus easily implementable. The 'Questionnaire' has to be developed accordingly, by experts who are guiding the conduct of employee surveys.

Hi,
If you read the book "Break all the Rules" you will get a very good insight on how to gauge employee engagement there is also some matter that deals with employee satisfaction. However, what most companies forget is that "employee engagement" is actually more important than employee satisfaction. Employee satisfaction is more related to the office atmosphere, the HR policies, Facilities provided to cool-off. In many cases companies think that the compensation is the most important factor for employee satisfaction, yes it is true to a very great extent, more so today than a few years back, but do you want satisfied employees ot fully engaged employees. Remember the level of "Satisfactory" in the ratings list. Does this level of employee get the best increment or is the "Excellent" employee. If that be the case organizations must gauge the level of Employee 'Engagement as well as the level as Satisfaction".

You can't satisfy employees for a long term by any monetary benefit. Human wish never end. Better is to engage the people in such an atmosphere were they will enjoy the work.
Work out on questionnaire that will serve the purpose of your organisation as the definition of satisfaction is different at different culture.
