We have started our office in the year 2007, and since then, I've been trying to complete the required formalities, but I have not received any response. Kindly assist me with the necessary formalities and forms.

It has been more than 5 years since the establishment started. If you are going for PT registration now, you need to pay the arrears from 2007 with a penalty.
Documents Required for Obtaining PT & Company Tax Assessment Numbers
Documents / Details required:
- Copy of MOU/MOA
- Rental agreement of Chennai branch
- List of employees & their total gross income (April to September & October to March)
Statutory / Licence Fees
- Deductions as per PT slab given by the corporation
- Company Tax – RS. 1000/-
- It should be paid once every six months
