Kindly take my words for granted "negative/bad habits can be treated as there is a 101% cure"
I had designed a PPT for STUDENTS(college students) and EMPLOYEES(all levels), as how they can "CHANGE THEIR BAD HABITS".....Its really effective and i had implemented in our organisation where i had succeed in changing habits of few employees......ITS A TIME CONSUMING PROCESS and it depends on the TRAINER, how dedicated he is, in producing the BEST OF THE BEST RESULTS by makings his trainee(s) to PERFORM THEIR BEST OF THE BEST...
I am just an ordinary guy who believes in doing EXTRA-ORDINARY things, only with the grace of ALMIGHTY.........
With profound regards

Dear Abhi,
I am sorry not to have understood your problem. The main problem is like most students, you have also sought solutions without giving enough information for us to help you. Had you stated that you are studying OB as part of your B.Tech. course, people would have got some idea.
One thing I learnt from my school days, is we learn better when people question us, rather than give answers on-a-plate. If you do not like being questioned, then please seek some other teacher.
In these days knowledge cn be acquired at the touch of a button, unlike our college days when even books were not available. Hence, i suggest that you search the web to findout what I was on about when i asked you the difference between items on a questionnaire and the type of scales.
Also, unless I know the aim and objectives of the survey, i can't help you.
Have you only got to design a questionnaire or conduct a survey and analyse the responses?
Have a nice day.
