What kind of letter should be given to an employee if his designation/job role is changed from sales manager to sales agent?

Sounds like a demotion,
A letter addressed to the affected employee from HR Head / Manager should work.
Mentioning change of role responsibilities (of new role) and expectations in new role.
Also specify if there is any change in his designation / employee grade or band / remuneration etc. Couple of motivating lines are suggested.

CC to his Department Head / his other peers (old and new department) / functional heads
