Dea All,
Greeting of the day !!!!!
I am working in manufacturing compnay .One of our old & good employee lost his Son last months due to some illness.Because kid is very serious they take him a Private hospital rather than ESI Hospital.Now he is no more.But our managment decided that ,there should be some ESI benefits to Employee,like Medicinal benifit ,funeral expensese and all.so I want to know from you people.that is eligible for any kind of benefit from ESI.
Pls let me know.

the insured person will get the death benefit and other benefits only ,treatment and and medical benefits will get for family..
see u cna get u r son medical expediencies as u have went to private hospital u take all the bills and take the forms from Esi and fill and submit to teh local dispensary u will get the amount back..

Dear Mr. Sharma.
Your employee can get medical reimbursement from ESIS authority . only he has to fill the form which is available at ESIS office . Ask the employee to fill the form . but Doctor has to certify that it was emergency and it was necessary at that time to treat him in private hospital. Basically it`s take long time . but you have to keep follow up with the authority. hope fully your employee will get reimbursement by mid of 2015, if it is found in order by the authority.
I have got the reimbursement for my son`s ( same scenario) treatment after two years . you have to regularly follow up with the authority .

You can get Funeral expenses of Rs 10,000/- by filling up Form 22. If you have the medical bills you can claim reimbursement .

Dear Mr. Sharma,
No need to worry about this situation, you are required to collect all the medical bills for medicines, checkup or body tests, which all duly certified by the hospitals doctors. in this situation you are required to collect the below mentioned documents.
1- Medical Bills
2- Reports & Reports Bill
3- Emergency certificate from Hospital (M.S.)
4- Certificate B from ESIC Dispensary
5- Covering letter( Including the full story of treatment to dead, amount of expenses, and exact situation)
6- copy of cancelled cheque
7- Copy of ESIC card
8- All documents should be attested by the hospital (M.S.).
Hope your employee will get the full reimbursement of expenses if s/he is filing proper documentation alongwith proper follow up with dispensary & DMD office (Tilak Vihar).
