Hello Friend,
I am also facing the same kind of problem. To overcome from this problem we have to work hard then only we can overcome.
I am giving some tips to over come from this problem it may helps to you some extent.
These are all very common tips for everybody but if we follow i am sure we can enhance our communication skills a lot.
what you need to do is :
1) Start speaking with your friends or colleagues who have sound knowledge in English and ask them to correct your language.
2) Try to read Interesting English articles like Mag zines, Comic Books etc.. and ready English Newspapers, make it as a hobby.
3) Try to chat with your friends thru Messangers like Yahoo, G talk etc..
4) Try to draft sample Letters and Mails and show to your Friends and colleagues and ask them to correct it .while writing next time keep in mind their suggestions. It will develops your written skills.
5) watch English news daily in various channels and observe how they are talking it will increase your english presentation skills and vocabulary
6) if you possible try to join any speaking English Institute it may helpful to you some extent.
apart from above there are lot of ways to improve our communication skills.
one thing is true today's competitive market communication skills are playing major role to get good gob.
any way I wish you all the best , If you plan systematically toward your communication skills I am sure you will find tremendous results in very short time.

There are many suggestions above but the key to your problem is in you adopting some of the numerous alternatives and beginning today. It wont be easy initially and you will get disheartened and disappointed at times. However if you devote a fixed time and have a clear plan in mind you will soon notice the improvement and you will be motivated.
Here are few practical tips for your ready reference.:
The first step is to talk only in English. Never mind the stammer, the long pauses when you fumble for the right word and ignore the laughter of people when you use the wrong word or mess up your pronunciation.
The simultaneous step is to read as much as possible in English. There will be many new words that you will come across. First attempt to derive the meaning by reading the whole sentence. Then confirm your understanding by checking in a dictionary. Many words in the English language can have more than one meaning and can be used in various contexts. Eg. Eye is also used to refer to the small opening in a needle through which you thread it or bark is also the outer covering of a tree.
Whenever possible move around and interact with people who speak good English. We often pick up the incorrect words or pronunciation or slang simply because we interact more with people from similar background to our own who might be as weak as us in English.
If you want to improve your vocabulary, you must try not to repeat the same word in the same sentence or subsequent sentences. Use the synonyms of the words. Use a Thesaurus for the same. When making notes in college you can attempt to paraphrase the lecturers statements and use your own words to write it down.

You first buy a Rapid-ex English Speaking course book in your mother tongue.
Try to understand the basic form that "I go-I am Going -I have gone-I have been going" from any child's easy grammar book.
Read any thing written in English loudly preferably sitting in front of dressing table mirror min 30 minutes a day.
Try to cover up the rapid-ex course max within 45 days.
Whenever you want to speak anything in English just take a minute to think the same in your mother tongue and then translate the same in English.

When we try to improve our English Communication Skill we shoul do it in a fun way. Enjoy the learning process and don't take it as a lesson in class.
The suggetion from our friends above are great ways o learn English...as for me I would add my other methods of learning English (which really helps me to speak English and still having fun). Here is some tips that may be useful for you :
1) Watching English Movies with English subtitles will also help you to improve your English
Communication Skill :-) You can learn the vocabularies and how they speak of it. Please
never use other subtitles beside English and you should be watching English Movies. If you
find some difficulties in understanding the languange you must open your ENGLISH to
ENGLISh dictionary, don't use English to other languange dictionary.
2) Play some English Languange Game such as Text Twist, Bookworm, Scrabble, etc. which will
help you to learn vocabularies and remember them in your mind.
3) Do some English quizes such as one in the Reader's Digest Magazine.
4) Find a friend abroad using the internet and do a lot of chatting with them (using English).
5) Do some translation from English to your mother languange and vice versa. Try to
summarize a book or article from your languange to English or from some English book to
your languange. Type it in Ms Words or Ms Powerpoint and share it with others too. This
way you will learn English faster and you can also make people happy with your published
articles.
6) Listen to English songs and try write down the lyrics by listening. If you have wrote it
down, crosscheck the lyrics with the lyrics published in internet. You can find how may
mistake that you've done and learn not to do it again. After you get the lyrics correct you
can try to find the meaning or translate it into your mother languange. Later on you can try
to remember the English lyrics and the songs then sing it on your way to the office :-)
Try this method everyday and I assure you will be able to speak English fluently as sson as possible. And you will also be happier in your life :-D

I am sorry to disagree with the advice given by the Banerjies. One of the problems with people whose first language is NOT English, is their tendency to think in their mother tounge and then translate into English.

Suggestions of knowledgable friend, even my own on Citeman only shows how to go about it. But You are the person who has to shake off hesitation, fear and a consciousness of your limitation. Be confident and bold and start talking, firstly may be with faltering errors and slowly gaining fluency. Here confidence in acjievemnt is essential to begin with and once you start talking more comfotabily you will feel more confident. Its a rigmarole. So, tighten up your belt and begin ....
Its like a child learning to walk & talk.
Then certainly hooking to a friend who can converse in English comfortbly will be a boon. But dont wait to find one, begin now,
Tell yourself, " Ican do it" and like a magic it will help you move on.
The problem of good or bad English comes later, first begin ... for communication is important. In this besides language, body language and gestures have a role to play.
In the industry I have seen many upward moving persons with very little formal education gaining comfotable level of communication in English with necessity and motivation.
