Hello Seniors , juniors and peers...

THis is vikas khatter..

i have a problen in esi ip no. transfer...

one of our employee has two IP no...

actually i joined my present company...

i register employee in esi portal and give him TIC after generating.

I did not know that the employee has worked here before....he has worked in our organisation main code of esi registered in faridabad....

now he joined in our sub unit registered in okhla, delhi....(esi sub code)

He has done the formality of photo by old no. but he did not go to esi branch office to collect the smart card.....when he joined again in our sub unit then he did not tell me i have already the esi no. as matter of fact i was also new in that organisation....

when he went to esi branch office delhi for new esi no. photo formality and he successfully done...but when ask esi personnel for new esi generated smart card then they told him that you have 2 esi no. you can not be given smart card by new esi no.

ask to your HR transfer the new esi no. contribution amount to old esi no...

his new esi no. date of joining 01/04/2013....

now i have insert his old no. in esic portal.

and esic personnel told me to show his date of joining of old no. as per 01/04/2013..and write the transfer letter on letter head....

now is my concern is that i have been asked to fill the contribution by old esi no. no need to fill employee contribution by new no.

so should i show zero days contribution on new esi no. by showing leave reason. and when can i show left the new esi no.

as we know the esi contribution period is march-september...

when can i prepare and send the transfer letter to esic branch office....

please suggest me what kind of letter should i draft for this...is there any form to to write a letter........

Better way not to fill contribution in new IP and let his old number continued. It ll be helpful to the employee as well to get benefit from ESIC hospital in case of emergency. After 3 months the validity of his new IP will be expired automatically. Just take care to not to fill contribution in both the IPs

Sorry for some wrong inf. Actually that IP number is valid life time so you should fill the contribution in Old number only and take suggestion from your consultant if you have. and also visit the ESIC site for more information.

can you provide me the details which form should i use to transfer the new ip contribution into old id no...
Apart from this i would like to know that if i fill this month contribution in old no. then should i show zero days contribution in new ip no by disclosing leave reason....
please suggest me...

will it be a perfect to print this annexure on letter head
