First gather some information about what facilities the company has and conduct a survey of employees about what they think about the facilities. Also gather demographic information about their age, sex, status in the company, etc. These will be your independent variables. While asking for their views you need to ask them how important those facilities are for either carrying out their work or satisfying their social/personal needs.
Kindly search the web to see what others have already done and how on smilar studies.

Will you be conducting an interview using the structured questionnaire? In any case, I suggest that you leave the demographic and personal details till the end.
Read the other questions again and see if the same responses like "strongly agree" are appropriate for the statements made or would it better if another response was used or the statements altered. For example, compare items 5 and 6 to see which is worded better and makes sense.
