I have worked in an ltd. orgz for 3 and 1/2 year and left that orgz on 16th nov.2009. now i want to withdrawal my PF and EPF amount. I want to know
1) am i eligible only for PF amount and my EPF amount will waste.
2) I do't have the latest slip. so how i came to know about me actual money.
3) what is my total amount of PF and EPF as we have no receipt of EPF amount.
4) are the receipts of PF required to shown while getting withdrawn.
5) I have to fill 2 separate form or only 1 form to withdrawal is sufficient. which form is applicable.

you have to fillup the following forms and submitt them in PF office.
Form-19, Form-10(c),Form-10, Form-5,Form 3-A (Revised)
To find out the total earned money you have to go through the salary sheets of your previouse organisation. They should help you.

I have same problem like Mr.manish.I fill up Form-19 & Form-10C.Other form given by you are necessary to give or that are supporting documents.(Form 10,Form 5 & Form 3A).
Please give advice...

You are eligible for both PF and EPS amounts. If you have a service of 3 years and 6 months or above, it will be rounded up as 4 years. If it is below 3 years and 6 months, it will be rounded down to 3 years. The factor stipulated in Table D for 3 years is 2.98 and for 4 years is 3.99. This will be multiplied by the average salary of last twelve months to get the withdrawal benefit towards EPS / Pensoion Fund.

Generally EPF contribution is 12% each by both employee and employer including EPS. But in some typical cases it will be 10% each. Abbas.P.S
