ESI Form 5 xls finally

Hi, After searching citehr and google i spent around 1 hour to format this xls. Hope it is helpful. I did not check the contents and grabbed the same from ESI-Returns Form 5.doc.

Please check and use the same.

I am unable to upload the file. I have tried number of times. I can mail the same to someone who can upload it on this thread
