Please try to understand the concept and effect of diagnostic study and survey exercise you are aiming at. Secondly, the instrument you are going to use for finding is not only inadequate, but it is incomplete as well. Please speak to your management and get their concurrence, especially their ownership to make necessary corrections after the survey findings.
Most of the time, survey findings remain unsettled in your office like an untouchable thing, not even discussed by senior management.
